      Where to File IRB Approval Study Level Documents for Initial Approval?

      Document Type:
      IRB Initial Approval, Initial Review Submission forms & Continue Review Report?

      Looking at the TMF RM Index. IRB Approval Document Type- Country level & Site level only.

      Please advise.
